Fulfilling Required Documents

When uploading and selecting "fulfill existing requirement" the document uploaded is not actually filling the requirement (i.e. Daniel Kong - CV). The data that I am entering like the dates and notes are also not saving on the initial upload.

License Number in Basic Info

Is it possible to add the license numbers to the basic info section?

Would love to see a separate tab to store third-party logins in the provider profile.
