Can we add the document type "Case Log" or "Procedure Log)?

Can we add the following two document types: Professional Liability Response (provider's explanation of a malpractice/professional liability claim) Adverse Action Response (provider's explanation of a license discipline, privilege revocation, DEA sanction, peer review action, etc.) These could both live under a new parent category, "Disclosures & Reponses" or something similar

(see Robert Armbruster for example) - need to be able to accommodate both the registration/certificate and the verification evidence (like we do for DEA). Workaround was just adding two 'State Controlled Substance Registration' and one has a (1) at the end, so you can't tell from the doc screen which is the certificate and which is the verification evidence

So it would be, e.g., "Allopathic Physician (MD)" and Osteopathic Physician (DO)" (adding space between physician and MD/DO)
